Brain Injury Out-Patient Rehabilitation

This is a structured, coordinated, comprehensive, interdisciplinary out-patient program for people with brain injuries, neurological disorders from disease, illness, or learning disabilities.

Admission Criteria:
1. Medical stability
2. Functional mobility
3. Free from risk of harming self/others
4. Free from alcohol & substance abuse or willing to actively participate in
alcohol/substance abuse program
5. Desire to achieve agreed upon goals
6. 16 and older
7. Means of funding

Discharge Criteria:
1. Increased level of independence in the home
2. Increased level of independence in the community
3. Return to school
4. Return to work

Services Available:
1. Occupational Therapy
2. Physical Therapy
3. Speech Therapy
4. Cognitive Treatment
5. Vocational Treatment
6. Life Skills Training
7. Neuropsychological Assessment
8. Neuropsychological Consult
9. Neuropsychological Treatment
10. Case Coordinator
11. Respite
12. ABS Assessment
13. ABS Counseling
14. Medical Services
15. Transport

Home & Community (Outreach) Services

Services for people with brain injuries, neurological disorders from disease or illness, or learning disabilities are designed to achieve predicted outcomes. The focus is on home and community integration, socialization, and functional activities.

Admission Criteria:
1. Medical stability
2. Functional mobility
3. Free from risk of harming self/others
4. Free from alcohol & substance abuse or willing to actively participate in
alcohol/substance abuse program
5. Desire to achieve agreed upon goals
6. 16 and older
7. Means of funding

Discharge Criteria:
1. Increased level of independence in the home
2. Increased level of independence in the community
3. Return to school
4. Return to work

Residential Rehabilitation Services

Structured program for people with brain injuries, neurological disorders, or learning disabilities. The program coordinates with client, family, and support system to enable persons to live at home and interact in the community.

Admission Criteria:

4. Free from alcohol & substance abuse or willing to actively participate in
alcohol/substance abuse program
5. Desire to achieve agreed upon goals
6. 16 and older
7. Means of funding

Long Term Residential Services

Designed for the need of persons with brain injuries, neurological disorders from disease or illness or learning disabilities. The services maximize stability in functional and social performance and promote health and safety awareness.

Admission Criteria:
1. Medical stability
2. Functional mobility
3. Free from risk of harming self/others
4. Free from alcohol & substance abuse or willing to actively participate in
alcohol/substance abuse program
5. Desire to achieve agreed upon goals
6. 16 and older
7. Means of funding

Discharge Criteria:
1. Retain highest level of idependence and social interaction possible in the least restrictive environment in the home and community

Admission Criteria:
1. Medical stability
2. Functional mobility
3. Free from risk of harming self/others
4. Free from alcohol & substance abuse or willing to actively participate in
alcohol/substance abuse program
5. Desire to achieve agreed upon goals
6. 16 and older
7. Means of funding

Discharge Criteria:
1. Return to work (part time, full-time, volunteer)

1. Vocational Treatment
2. Neuropsychological Consult
3. Neuropsychological Treamnt
4. Neuropsychological Assement
5. Life Skills Training
6. Job Coaching

All other services are available and can be arranged as needed
